Dear Doctor and Staff,

We are pleased to announce that our Oncology Patient-Centered Medical Home® model of care for patients with cancer and blood disorders continues to be locally and nationally recognized (NCQA, ASCO) for creating a new value proposition for patients, referring physicians and payers. Timely coordinated inpatient and outpatient consultations with clearly defined responsibilities and expectations between our physician-led team and referring primary, surgical and specialty physicians and their staff are the hallmark of this innovative model. A referral to one of our three sites provides an entire system of support that snaps into place to ensure that each patient receives the evaluation and treatment that he/she needs via consistent: patient engagement, education, access, navigation, and care coordination/communication with all involved physicians.

One national and two regional payers in southeastern Pennsylvania have responded to our superior Patient-Centric outcomes. Ours is the only hematology and oncology practice in the region to secure this type of patient-centered contractual agreement with Keystone-First, Aetna, and Independence Blue Cross. This is in acknowledgement that we have demonstrated delivery of exceptional quality of care based on national/local benchmarks in addition to our success in reducing unnecessary resource utilization by keeping patients healthier (less ER and hospital admissions, etc.) during their treatment. Our application of PCMH principles to hematology and oncology care has been embraced broadly and is being implemented in hematology and oncology practices and cancer programs across the country.
